Would this be appropriate for an adult with dementia?

Dementia is much more complicated than most people realize. To reduce the rate at which dementia patients loose abilities, all senses must be stimulated. For example, as your hearing decreases, your math skills fall off rapidly. Spatial recognition can be stimulated by this hexagon puzzle, but by itself it won't contribute much. I'm 76 and to keep sharp I play computer chess, computer cribbage, sudoku, I watch lectures from the greatcourses on line service, I listen to music, and I build kit airplanes. I bought this puzzle for my very bright 3 1/2 year old grandson. My son sets the puzzle up and the youngster must finish it matching the layout provided. I sent my grandson a lot of stuff while the virus has everyone staying home. It kept him busy, happy, learning everyday, and took a load off my son. Again, your dementia person should have someone to work with on this and other task type aids.

It is a 2 player game?

It is not made as a 2 player game, but I do it with my kid all the time. She is young and needs help completing it, but it was fun working on it together.

One side of piece had marked hexagon and other size does not. We have to use some pieces reverse side to make pattern provided. is this expected?

Yes I believe sometimes you will flip them over to make them fit. We haven't done all the solutions yet but I do remember flipping pieces around to make them fit. I guess I didn't pay much attention to whether the hexagon lines were showing or not.
