Q: my current box is protruding from the drywall 1/4". Can this be used to cover the electrical box? Then a plate mounted to the front?

No. This attaches to the same screws the outlet does, extending the depth of the existing box. It does not extend around the sides. What you need to find is a deeper than usual outlet cover, preferably also oversized.

Q: Do you have to disconnect the existing outlet wiring to install or can it be angled over the outlet?

I used this extender for a switch (vs an outlet) and was able to install the extender without disconnecting the wiring. You will have to remove the outlet screws and pull the outlet out, before re-inserting over the extender; but the wires should be able to remain connected.

Q: Can these be stacked .

Probably (although I haven't tried). These units are 5/8" deep, while the included screws are 1.5" long. Keep in mind these units are slightly tapered on the side, not flat, so 2 stacked would look a bit weird.
