A medium really is a medium (if not a tad smaller), etc. Since it is a thin cotton material there's no give so be sure to order up particularly if your shoulders are a little wider than average or you need more room for your bust. I wear a Wal-mart size S shirt, but usually an M with other stores, or even an L from Forever 21. I ordered an L and an XL in these. The XL is slightly loose, and the L is spot on. The fabric is like a cool thin sheet type material, not scratchy. I bought a navy blue and a maroon. The navy was nice and dark, but the maroon was more like "brick".
