After much research I discovered that I could purchase the sim card here but it had to specifically be this model (SIMGLW206R) or it would not fully work with the Samsung Galaxy S5. Once I purchased and received the sim card I went to the Boost Mobile dealer and they were able to activate and set up the account within minutes. My phone has absolutely no issues and I am able to receive LTE signals, make phone calls, surf the web, etc without any problem.
