Question 1

When calibrating, should i wait until the pen reaches the ph the solution should be before i go to the next step. or when the smiley face pops up?

Smiley face. That said, when taking readings, even in the calibration solution, remember to swirl the meter in the solution with some briskness. I found that my pH pen continues to walk down or walk up almost a half a PH point just from swirling it in the solution.

Question 2

So does this ph tester come pre calibrated? Thats what the user manual makes it sound like. I already have the 7 & 9 range just not 10.

The base pen comes with enough solution to last at least a year of biweekly checking/  calibrating. The full kit comes with big bottles of all 3 ph ranges and a storage solution. I havnt even had to open my large bottles.  But I plan to next month just to exchange my small bottles (I've used for almost a year now) 
I have maybe had to calibrate 2 times and that was just to be safe. My water stays the same so I know what it is before and after my nutrient feeds so if it ever reads off, I just check it on a solution for the proper readout.

Question 3

Anybody had success using this for fermentation? I'm hoping to find one that's easy to use/store that I can use for fermenting hot sauces, etc.

I use mine specifically for Horticulture and I have zero experience in fermentation; that said, I believe this will still be a great tool for your purposes.

Question 4

Can this tester be used to test ph for medical / intravenous solutions? i am looking for something that is accurate to ~0.1ph. any experience?

It does measure to the tenth (0.0).  The other Apera model PC60, measures to the hundredth (0.00).

I use the PH20 and noticed when making changes to your solution, change down reads fairly quickly. Changes to raise PH reads more slowly, so give it a few minutes to stabilize the reading.

Also I use the 7.0 calibration fluid in the cap when storing between uses.  Helps notice if recalibration is needed.
Always rinse and dry probe before storing or testing.
