Can it be laid flat under my neck when I watch tv or is it always U curved?

If you undo the latch it will go straight. I bought one for my sister and she used it after surgery straight to keep the seatbelt off her inscision and she uses as a neck pillow and in all other positions. It's super comfortable and she loves it. Been using it for a year or more. I am thinking about getting me one.

Can the strap be opened to allow more length?

Yes. It has a plastic snap/« buckle » and the strap is adjustable. I actually use the pillow in the open position because it is so versatile.
