Can I stick this inside my Otterbox rather than on the phone itself? Also, are there two sets included in this package?

No you can stick it to your case. I bought a few because i like to switch up my cases but didn't want it on my phone so i put one in each one of my cases works better for me this way

How is the adjustable necks strap secure? It's not one continuous loop and rope slides through the plastic length adjuster without pressing the button

Honestly, I use a regular lanyard with mine that clips to the little loop on the phone with a small caribener. The neck strap was not long enough for my needs.

How do I use the neck lanyard?

The lanyard goes through the carabiner on the tag on your phone. I use this daily when I have no pockets.
