Does this cartridge include a fine mesh on the input and output?

It comes with sponges. But they clog fast if your using fine carbon or such i suggest media bags.

Filled with polyphosphate or siliphos would this be a fair size for a 2.5 bath townhouse?

I have the same filter housing filled with 500 grams of Siliphos balls. The change in water quality and hard water build-up was immediate. The filter provides full house treatment for a two bedroom 2 bath home for two adults. I replace the Siliphos every 6 months to maintain water treatment quality.

Where does the foam piece go? Top or bottom of filter? Thanks!
