Will this replace a amp research replacement motor (80-03129-90) (80-03129-90)?

I believe so. They look similar. The holes look like they would line up. Seen one on ebay taken apart and it looked just like my old one, which I had taken apart.

How do you hook this up to 12v dc?

One wire is positive (hot) The other wire is a ground. The motor will run one direction. If you switch the wires it will run the other direction. Hope this helps.

Does the motor include the gear sprocket as shown in all of the pictures?

It does include the gear. It comes exactly how it’s shown in the picture. At least mine did.
