I need a set of adjustable weights so i can have 3 lbs and 5 lbs for each ankle. this is what my pt recommended. will one set adjust to both weights?

No pics on wrist.. will they adjust small / tight enough to fit wrjst?

Yes, although you might have to remove one of the one-pound weights to accommodate the smaller diameter if your wrists are very small. They fit my wrists just fine.

If you remove weights and then need to put it back on, do these weights fall out? or do they stay put?

I already have several sets of ankle/wrist weights. These are really high quality and comfortable. I got two five pound weights as i felt that i could lower to four or three (neither of which i have) and add to in addition to my other five pound weights if i wanted to. the little bags coming out is one of the things i was looking for and there is a bit of a stretch to the strap which helps make them fit nicely.
