Does this come in a cardboard holder that holds the mirror in place?

No, there is no holder. It has 3 small silicone suction cups on the back to stick to a window or mirror. I would also say to get a 15X mirror because this one has quite a bit of distortion at the edges so you can only view things well in the center.

What is v Diameter of mirror?

Mine is 3.5" in diameter. I think the width is limited by the depth of the concavity, which equation seems to allow it to magnify to that extent, though I have not looked this up and am basing it on hazy memory of college knowledge (a clunky and accidental rhyme!) and the fact that I have not yet seen any wider ones for sale. It works very well, though you have to hold it close to your face and it takes some getting used to as you have to find the right angle and distance. I love it but I also wonder if it is making me fanatical about my pores as you really can see a LOT with it. I can use it on the very rare occasions when I put on makeup though you can see only one eye at a time. It is also very durable. I have had it for many years and have dropped it a number if times but it has never even cracked. I hope this helps!

Will suction stick to painted wall?

if the surface is clean and smooth it will. best result are when you damping the suction cups with a wet watered cloth or towel.
