Is this swim ring 20 inches from one side to the other side or is the 20 inches the hole in the middle?

20" from one side to the other. It is for children 3 to 6 yrs. 20" hole would not hold a child in the middle.

I can see that one side is printed. What about the other side? Is it printed or plain?

Both side printed but don't go for it

What is the diameter of the opening on the inside"

Very small! This is for babies I would say.
