It says "Easily transfers between 1.25” and 2” hitch receivers" Does that mean it includes an adapter? If so, is the adapter for a 1.25 or 2 inch?

Just wanted to point out that this bike rack is best if you have a 1.25" hitch. In a 2" hitch, the adapter is in one corner of receiver and when tightened/installed, the rack was tilted a bit for me. Not a real issue I guess but a deal breaker for me as it drove me crazy seeing it tilted in the rearview.

will this work with RV

It will certainly fit into he receiver hitch but as far as the clearance behind the RV, you will need to measure.

can you add an additional lock to this? That cable lock can be cut into in a second.

Depending on where we are we sometimes use a second cable. It's easy to do. We mostly do this when the bikes are on the rack overnight.

Does this work with cars that have a spare tire on the back?

Thanks for checking with Thule. You would need a hitch extender to make this work. Such product are not made by Thule - you can check with U-Haul to see if they have it.
