Is the 20mm bit reduced on the other end?

Yes this bit is reduced to 3/8” on one end.

will the 7mm work for slim line pen blanks?

Yes it will as long as that slim line pen kit requires a 7mm drill bit which most do.

Will this work on plastic?

I believe this bit could drill plastic if you start the bit slowly...if you could secure the piece in a vice it would be safer to drill.
