I'd guess it stays very hot for maybe ~90 seconds, then hot for another ~2 minutes, and then warm for another ~8 minutes. Very disappointing. I haven't tried it in the freezer. **Q: Why does it smell like amonia fresh out of the bag?** A: I don't think it has an amonia smell, It has a oder of some kind but nothing offense. It certainly helped my sore elbow. **Q: How long should I freeze this ice sleeve before using?** A: No matter how long you freeze it, it won't stay cold for more than 10 minutes. I'm not all that small of a woman. but... The small cuff fitted on arm, is not all that snug. The small is good fit on leg. Use for muscle cramps and pain. Heated in microwave close. to max of 45 sec advised, and cuff was not all that hot and lost warmth within the 15 minutes when used. If I were designing this, I'd like the size to be longer and maybe offer an x-small (which I may have overlooked when ordering). Good addition in my the therapy tools. Have not used the hand therapy yet, but fitted (one size offered) again runs large too.
