Q: Can this be worn horizontally on your belt?

Yes, this has two Alan screws you can do vertical or horizontal and flip it over for left or right hand.

Q: How does the belt clip work?

It comes with two guides. I only use one. I use the top of the clip and one of the guides. Then slide your belt in between the guide and the top of the clip. Depending on how wide your belt is adjust the lower guide to fit. You're able to adjust the height and position of the knife with the two screws on the back of the clip. I've had this knife for almost a year.
